POS 52 is a psychiatric facility providing partial hospitalization. A partial hospitalization programme sits between weekly outpatient therapy and inpatient admission: the patient attends a structured, intensive programme during the day and goes home at night.
That middle position is exactly why the code gets miscoded. The patient is not admitted, so the biller reaches for an outpatient place of service. But the programme is hospital-level care delivered by a facility, and the payment rules follow the facility, not the calendar.
| Code | Setting | Patient stays overnight? |
|---|---|---|
| POS 51 | Inpatient psychiatric facility | Yes — 24-hour care |
| POS 52 | Psychiatric facility, partial hospitalization | No — structured day programme |
| POS 53 | Community mental health center | No — outpatient CMHC services |
The line that matters for 52 against 53 is not the intensity of the care but who is delivering it. A community mental health center running a day programme is still a CMHC. A psychiatric facility running one is POS 52. Read the entity, not the schedule.
POS 52 pays the facility rate. The professional fee is deliberately lower than the office rate because the facility is separately reimbursed for the overhead a physician in private practice would otherwise carry — the room, the staff, the equipment.
Billing POS 11 on a service delivered inside a partial hospitalization programme therefore pays more than it should. It is not a clever workaround; it is an overpayment sitting on the books waiting to be identified and recouped, usually with interest and usually across every claim in the series rather than the one that got noticed.
The reverse error costs you money quietly. Billing a facility place of service for work genuinely performed in an office setting takes the lower rate on every line, and nothing in the remittance flags it.
A PHP is not simply frequent outpatient therapy. Medicare expects physician certification that, without the programme, the patient would require inpatient psychiatric admission — the programme exists as an alternative to admission, and the documentation has to show that.
Programmes are also expected to meet a minimum weekly intensity, commonly cited as at least 20 hours per week of therapeutic services. Both the certification requirements and the hour expectations are set federally and revised periodically, so confirm the current rules with your MAC before relying on them for a specific claim.
This is the error that costs PHP providers the most, and it is structural rather than clerical. A partial hospitalization day generates two separate billing streams:
Collapsing the two, or putting the professional service on the facility claim, produces denials that look like place-of-service problems and are actually claim-form problems. If you are seeing CO-58 on PHP claims, check this split before changing any codes.
POS 53 — community mental health center and the full place of service code list. Not sure which applies? The POS code decision tree returns the right code from two questions. For the service line as a whole, see mental health billing.
Stepping up from a day programme? POS 51 — inpatient psychiatric facility covers formal admission and 24-hour care.
PHP billing splits, place of service and the facility rate are exactly where behavioural health revenue leaks. We work it end to end, and the first week is free.
