RVU Values for CPT Codes in 2026

Work, practice expense and malpractice RVUs for the most common codes, taken straight from the CMS 2026 file, and what each number means for pay.

How Many RVUs Is 99213 or 99214?

Under the 2026 Medicare Physician Fee Schedule, CPT 99213 carries 1.30 work RVUs and 99214 carries 1.92 work RVUs. Add the practice expense and malpractice RVUs, and the office totals are 2.85 RVUs for 99213 and 4.06 RVUs for 99214. In a facility they are 1.72 and 2.53. At the 2026 national conversion factor of $33.4009, that works out to $95.19 and $135.61 in an office, before any local adjustment.

Most people searching for “99214 RVU” want one of these numbers. Which one depends on who is asking. A doctor paid on productivity cares about the work RVU. A practice estimating Medicare income cares about the total RVU for the right setting. Both come from the same CMS file, and you will find both below.

2026 RVU Values for Common CPT Codes

These are the national 2026 values from the CMS relative value file (RVU26D, the October release). “Office” means the non-facility total. “Facility” means the hospital or surgery center total. NA means CMS says the service is rarely or never done in that setting. Click any code to see its price in your own Medicare area.

Three things stand out. Going from 99213 to 99214 adds 0.62 work RVUs, and going from 99214 to 99215 adds 0.88. The office total for a procedure like 11102 is about three times its facility total, because in an office the practice pays for staff, supplies and equipment. And therapy, preventive and test codes like 97110, G0439 and 93000 show NA in the facility column.

What an RVU Actually Measures

An RVU (relative value unit) is how Medicare says what one service is worth compared with all the others. Every paid code has three:

1
Work RVU (wRVU). The clinician’s time, skill, effort and the risk to the patient. It is the same in an office or a hospital.
2
Practice expense RVU (PE). The cost of running the place: clinical staff, supplies, equipment and space. It has two values. The non-facility value is used in an office, where the practice pays those costs. The facility value is lower, because the hospital or surgery center bills its own fee.
3
Malpractice RVU (MP). The cost of malpractice insurance for the service. For most codes it is the smallest of the three.

An AMA committee of doctors, called the RUC, surveys clinicians and suggests values. CMS reviews them and sets the final numbers each year in its fee schedule rule. That is why the values can change every January, and sometimes mid-year when CMS corrects its quarterly file.

Work RVUs vs Total RVUs: Which One Do You Need?

If you are…UseWhy
A physician on a productivity contractWork RVUsMany employed doctors are paid a set rate per wRVU. The other two RVUs are not your work
A practice estimating Medicare revenueTotal RVUs for the place of servicePayment uses all three parts, and the place of service decides which practice expense value counts
Negotiating a commercial contractTotal RVUs and the Medicare rateMany commercial rates are a percentage of Medicare, so Medicare is the benchmark
Benchmarking coding patternsWork RVUs per visitAverage wRVUs per visit show if visit levels are drifting up or down

There is no public “right” dollar amount per work RVU. Each contract sets its own rate, often using private salary surveys. The RVU itself is public, which is why the same 1.92 for 99214 comes up in every contract talk.

How RVUs Turn Into a Medicare Payment

Medicare adjusts each part for local costs using a Geographic Practice Cost Index (GPCI). Then it adds them up and multiplies by the conversion factor:

Payment = [(Work RVU × Work GPCI) + (PE RVU × PE GPCI) + (MP RVU × MP GPCI)] × Conversion factor

Take 99214 in a Manhattan office. The 2026 GPCIs there are 1.064, 1.162 and 1.586. So (1.92 × 1.064) + (2.00 × 1.162) + (0.14 × 1.586) = 4.59 adjusted RVUs. Times $33.4009, that is $153.27, compared with $135.61 nationally. Same code, same RVUs, different money. That is why a national figure is only a starting point. After the Part B deductible, Medicare pays 80% of it, minus the 2% sequestration cut, and the patient owes the other 20%.

Our RVU calculator and 2026 Medicare fee lookup does this math for any code in any of the 109 Medicare areas. It also shows the facility rate, the non-participating amount and the imaging cap.

What Changed for 2026

Two changes in 2026 affect how RVUs turn into money. First, CMS set two conversion factors for the first time: $33.4009 for most clinicians and $33.5675 for Qualifying APM Participants. Second, CMS made a 2.5% “efficiency adjustment”. It lowered work RVUs for most services that are not based on time, such as procedures, imaging and tests. Time-based services like office visits were not cut. So if a procedure’s 2026 work RVU looks a little lower than last year’s, this is usually why.

Where to Find the RVUs for Any Code

1
Enter the code in our 2026 Medicare fee lookup. It shows all three RVUs, both settings and the price for your area.
2
For the raw data, CMS posts the full relative value file (the PPRRVU file) every quarter on its PFS Relative Value Files page. The 2026 October release is RVU26D.
3
For surgery codes, also check the global period with the global period calculator. The RVUs for a 90-day code already pay for the normal follow-up visits.

Frequently Asked Questions

How many RVUs is a 99214?
Under the 2026 Medicare Physician Fee Schedule, 99214 has 1.92 work RVUs and a total of 4.06 RVUs in an office (non-facility) setting, or 2.53 in a facility. That is $135.61 nationally in an office at the $33.4009 conversion factor. Our fee lookup prices it for your locality.
How many RVUs is a 99213?
99213 has 1.30 work RVUs in 2026, with 2.85 total RVUs in an office and 1.72 in a facility. Nationally that is $95.19 in an office.
What is a work RVU (wRVU)?
The work RVU is the part of a code's value that reflects the clinician's time, skill, effort and patient risk. It is the same in every setting, and it is the number many doctor productivity contracts pay on.
What is the difference between facility and non-facility RVUs?
Only the practice expense RVU changes. The non-facility value applies when the service is performed in an office, where the practice pays for staff, supplies and equipment. The facility value is lower because a hospital or surgery center bills separately for those costs. The place of service code on the claim decides which one Medicare uses.
How do I convert RVUs to dollars?
Multiply each RVU component by its geographic practice cost index, add them together and multiply by the conversion factor, which is $33.4009 in 2026 for most clinicians. Without geographic adjustment, total RVUs times the conversion factor gives the national amount.
Do commercial insurers use RVUs?
Many do indirectly. Commercial fee schedules are often negotiated as a percentage of Medicare, which is itself built from RVUs, so the Medicare RVUs remain the reference point even when the conversion factor is different.
